Project Goal: To establish a training program for Family Medicine residents and clinical psychology students so they can learn to deliver integrated health care services as a team to underserved patients in a primary care out-patient setting.

Project Goal: To create a proposal to pilot an accessible treatment model for residents of Permanent Supportive Housing (PSH) with opioid use disorders through collaboration with on-site nursing staff.

Project Goal: To decrease the repeat utilization of high cost services and poor outcomes for patients using the behavioral health crisis system.

Project Goal: To pilot a payer-agnostic Collaborative Care program for depression in three UCSF Primary Care Clinics that utilizes patient identification for enrollment, systematic tracking with the use of a registry, brief psychotherapy, and psychiatric consultation.

Project Goal: To improve access to behavioral health services by providing educational outreach to primary care teams and implementing consistent use of screening tools.

Project Description: To develop and implement a Collaborative Care Model (CoCM) pilot that increases access to mental health services, improves patient outcomes for depression, increases patient and provider satisfaction, and is financially sustainable.

Project Goal: To increase access to behavioral health services by: (1) expanding the behavioral health provider (BHP) network, (2) using virtual behavioral health (BH) visits, and (3) adding mobile health (m-health) tools through a partnership with a health service technology vendor that functions as the central hub to facilitate referrals and match patients to the right care pathway.

Project Goal: To engage complex patients in outpatient care in the field who have high rates of inappropriate use of emergency room and inpatient facilities, but low rates of outpatient clinic care.

Project Goal: To pilot an embedded behavioral health treatment service in pediatrics that will support a patient’s ability to access mental health services in a timely and convenient manner – and ultimately lead to better mental health outcomes.

Project Goal: To offer comprehensive and accessible outpatient mental health treatments to the children and families receiving their medical care at UC San Diego Health.
