Project Goal: To decrease colorectal cancer (CRC) rates in patients evaluated at Department of Health Services medical centers by increasing CRC screening rates.

Project Goal: Building a public-private partnership between the Department of Public Health (DPH) and local youth workforce agencies to increase the diversity of applicants to DPH jobs, and improve economic conditions in San Francisco.

Project Goal: To develop a sustainable model of psychiatric outreach to individuals experiencing homelessness through the provision of psychiatric and substance use medications (medication-assisted treatment) in encampments and improved linkage to community health centers.

Project Goal: To develop Social Determinants of Health data governance infrastructure and an unifying informatics strategy in San Francisco Health Network and Department of Public Health to promote greater organizational alignment in addressing social determinants of health for our patients and populations.
