CIN Webinar – Building Processes to Improve SDOH Data Capture, Coding, and Referrals

Evidence shows that ethnicity, sexual orientation, and social determinants, such as access to housing and healthy food, are all important contributors to health, yet many health centers could improve their data capture of these factors to provide the most appropriate care to patients. Data capture of these measures is especially needed to risk stratify patients, so providers can tailor their care, predict patients’ future health, and encourage patients to take preventive measures. How can health centers effectively capture the social complexity of their patients through standardized data collection and connect patients to social care resources after a need is identified?

In this webinar hosted and led by the California Improvement Network (CIN), Dr. Reema Menezes and Jenna Lunger from Bay Area Community Health (BACH), a Federally Qualified Health Center (FQHC), and Khushdeep Grewal of the Community Health Center Network, a CIN partner organization and the health consortium in which BACH is a member health center, will engage in a discussion around improving social determinants of health (SDOH) data capture, coding, and referrals.

Participants will learn how BACH captures patient SDOH data through MyChart and other mechanisms. BACH will also share their work with technology provider OCHIN to map DHCS-prioritized z-codes to patient responses to screenings, and their work with FindHelp to streamline social care referrals to community-based organizations. Speakers will discuss their successes and challenges in improving SDOH data collection and how to find a standard solution that works across clinical sites.

This webinar is intended for providers, patient navigators and other patient support staff, allied health care providers, and health care leaders working in community health care settings.

During this webinar participants will:

  • Learn how to capture SDOH data using an appropriate data collection tool, such as MyChart.
  • Understand the facilitators and barriers to SDOH tool implementation, including coding and referrals to social care resources.

  • Khushdeep Grewal, medical director at the Community Health Center Network
